A type of Medicare Advantage plan built for people with specific needs, those on both Medicare and Medicaid, those with certain chronic conditions, or those living in a care facility. The benefits are tailored to fit.
A Special Needs Plan (SNP) is a Medicare Advantage plan that limits membership to people who share a specific situation, and then tailors its benefits, provider network, and drug coverage to that group. Because the plan is built around your needs, the extra benefits are often more generous than a standard Advantage plan, and many have very low or zero premiums.
You must meet the eligibility for the plan type, and you can enroll or switch outside the usual windows when you qualify.
For people who have both Medicare and Medicaid. Often the richest extra benefits and lowest cost.
For people with a qualifying chronic condition such as diabetes, heart disease, or COPD.
For people who live in a nursing home or need a similar level of care.
If you have both Medicare and Medicaid, a D-SNP can add benefits like extra dental, vision, hearing, transportation, and over-the-counter allowances, often at no premium. If you manage a chronic condition, a C-SNP shapes its network and drug list around that condition. We will confirm your eligibility and find the plan that fits, at no cost to you.

If you have Medicare and Medicaid, a chronic condition, or live in a care facility, you may qualify for richer benefits. We will check for you. No fees, no obligation, no pressure.
