According to KFF, Medicare Advantage plans made nearly 53 million prior authorization determinations in 2024, compared to roughly 625,000 in traditional Medicare, denying about 7.7% of those requests. Original Medicare almost never requires that extra approval step at all.

Two Paths, Same Destination

Medigap (Medicare Supplement) works alongside Original Medicare. Any doctor who accepts Medicare, nationwide, no referrals, no network. Costs are predictable, plans like Plan G cover nearly every gap Original Medicare leaves behind. You pay a separate Medigap premium and a separate Part D premium.

Medicare Advantage bundles Part A, B, and usually D into one plan, often with a $0 monthly premium. Many add dental, vision, or gym perks. The tradeoff is the one Gary ran into: networks, and prior authorization for many services.

Which One Actually Costs Less?

It depends on how much care you use. Medicare Advantage often wins month-to-month. But get seriously ill, and your exposure can climb to the plan’s out-of-pocket maximum, $9,250 in-network in 2026. Medigap costs more every month but caps that real-world risk far lower.

Can I Switch Later?

Will My Doctor Take It?

With Medigap, yes, as long as they accept Medicare. With Medicare Advantage, confirm your specific doctors and hospitals are in-network before you enroll, not after.
